Family Transition Service

In the transition service we provide person centred support for young people and families 14 to 25 across North Wales.

At CC4LD (Conwy Connect for Learning Disabilities), we are dedicated to providing comprehensive support to the youth across North Wales. Through three main work-streams, we aim to empower young individuals, enhance their knowledge, and facilitate smooth transitions into adulthood.

Our three primary areas of focus are Family drop-ins, Information Access, and 1-1 Support.

As a service we will look at 8 main areas:

    Education
